ALCC Improving Health Home Mission Grant
REQUESTS FOR PROPOSAL
Alabama Community Care- Region C, Inc. (ALCC, a Medicaid care coordination provider) created the Improving the Health Home Mission Grant to assist in the Alabama Medicaid quality of care provided to its recipients in Region C. As part of the program, ALCC is responsible for working with our network providers to develop work plans/projects that support the goals identified in the health home mission. As the regional coordinating organization, ALCC will accept and review applications from providers for proposed work plans/projects. Awards may be up to $300,000. No more than 40% of overall budget may be expended for salary expenses.
The intent of the program funding is to finance the implementation of work plans/projects that support at least one of the following health home objectives:
- Improved prevention and management of chronic disease;
- Improved access to and care coordination of health services;
- Reduce non-emergent use of emergency department.
ALCC is releasing the application February 2019, which must include, but not limited to, detailed information regarding the work plan/project rationale, work plan, budget and sustainability of the work plan/project. All applications must be received by 4:30 p.m. Monday, March 25, 2019. Applications must include a completed application template to be accepted by ALCC. A hard copy must be mailed to the following:
